Why is my monitor not running at its full refresh rate?
You paid for 120Hz or 144Hz and the computer is sending 60. The four things that decide what a screen actually runs at, in the order to check them, and why the cable is more often the culprit than the settings.

Short answer
Refresh rate is not automatic. Windows and macOS usually pick a safe default and leave it there, so the first thing to do is open the display settings and select the highest rate offered. If the high rate is not offered at all, the connection cannot carry it — that is nearly always the cable or the port, not the monitor. Check the monitor's own menu too, because some screens ship with their high refresh mode switched off.
A monitor advertised at 144Hz will show a perfectly good picture at 60Hz and give you no indication that anything is wrong. Nothing warns you. The screen does not complain, the computer reports success, and the only symptom is that the smoothness you paid for is missing — which is difficult to notice if you have never seen it working. That is why this is one of the most common quietly unresolved complaints in consumer computing. The chain from computer to screen has four links, each of which can silently cap the rate, and they need checking in a specific order because three of them are invisible from the settings screen.

- Understand what refresh rate is, so you know what you are looking for.It is how many times per second the screen is redrawn, measured in hertz. Sixty means sixty complete new images a second; 144 means 144. The difference shows in motion — dragging a window, scrolling a page, panning in a game — and not at all in a still image. If you are trying to judge whether it is working, move a window around rather than looking at a photograph.
- Set it in the operating system first, because it is free and it is often the whole answer.Both Windows and macOS have a display settings page with a refresh rate control, usually behind an advanced or scaling section, and it is set per screen. Exceed it and the high rate is simply not offered. A cable that works perfectly at 60Hz can be entirely incapable of the same resolution at 144Hz.
- Prefer DisplayPort where both ends have it.For a desktop computer driving a high-refresh monitor, DisplayPort is the path of least resistance and it is usually the connection the monitor's own documentation recommends. Where only HDMI is available, the HDMI version at both ends matters — the specifications define what each version can carry, and the manufacturer's stated maximum for your specific model is the figure to trust rather than any general claim.
- Swap the cable before you buy anything.Use the cable that came with the monitor, since it was chosen for that monitor's maximum. If it has been lost, treat an unlabelled cable from a drawer as suspect: cables are not interchangeable above ordinary resolutions and a great many sold as generic are built to an older specification. This one substitution resolves a large share of cases and costs nothing to try.

If everything else checks out, turn it off in both the monitor menu and the graphics settings, set a fixed refresh rate, then decide whether to turn it back on.
- Verify it, rather than assuming the setting took effect.The number in the settings box is what was requested. Confirm it is being delivered by looking at the monitor's own information display, which most monitors show in their menu and which reports the signal actually being received. That is the only reading that comes from the screen rather than from the computer.

- Assuming the monitor picks its own best mode — It does not. The computer chooses, and both operating systems have historically defaulted to a conservative rate. Buying a high-refresh screen and never opening the display settings is the single commonest reason for never seeing it work.
- Reusing an old cable from a drawer — Cables are built to specifications with real bandwidth limits, and above ordinary resolutions those limits bite. An old cable that works perfectly for a 60Hz picture may be physically incapable of carrying the same picture at a high rate.
- Plugging into the motherboard on a desktop with a graphics card — It usually works, which is what makes it hard to spot, and it routes the picture through the far less capable built-in graphics. Every display should be plugged into the card.
- Turning the resolution down to get a higher rate and leaving it there — Useful as a diagnostic — it confirms a bandwidth ceiling — and a bad permanent state, because a screen not running at its native resolution can never look sharp. Fix the connection instead.
If it doesn't work
Only 60Hz is offered in the settings
Cause: The connection cannot carry more at this resolution — Fix: Change the cable first, then the port, then check the monitor's port-mode setting. If a lower resolution suddenly offers higher rates, the bandwidth ceiling is confirmed and the cable or port is the thing to change.
The high rate is offered but reverts after a restart
Cause: A driver or a sleep-wake cycle resetting the mode — Fix: Update the graphics driver from the chip maker rather than through the operating system's update service, then set the rate again. If it still reverts, disable adaptive sync temporarily — it is the usual culprit for a mode that will not stick.
It works on one input and not the other
Cause: The two ports on the monitor have different capabilities — Fix: Normal, and documented in the monitor's manual. Use the port the manufacturer states supports the maximum rate, which on most high-refresh monitors is DisplayPort.
Smooth on mains, not on battery
Cause: Deliberate power saving — Fix: Look for a refresh-rate or adaptive-display option in the power settings and turn off the automatic reduction. Expect a shorter battery life, because that is what the setting is buying you.
Works through the laptop's own port, not through the dock
Cause: The dock or its connection is the bandwidth limit — Fix: Docks split a single connection between video, data and power. Reduce to one monitor, use the port the dock's documentation identifies as the fastest, and check what the laptop's USB-C port itself supports.
Everything reports the right number and it still looks the same
Cause: Nothing on screen is producing frames fast enough to show it — Fix: The screen redrawing faster changes nothing if the computer is only generating sixty images a second. In games this is the graphics card's frame rate; in video it is the source material, which is usually far below 60 anyway. A high refresh rate improves motion; it cannot invent it.

Is a high refresh rate actually worth having for ordinary work?
For scrolling and window dragging it is genuinely more comfortable, and some people notice it immediately while others cannot see it at all. For reading documents, spreadsheets and video calls it changes very little. It matters most for games and for anything involving continuous motion. Why does my monitor say 144Hz on the box but the maximum is lower over HDMI?
Because the maximum depends on the port. Manufacturers quote the highest figure the panel can achieve, which is usually available only over the best-specified input. The manual will state the maximum for each port, and it is common for HDMI and DisplayPort on the same monitor to differ.
Do I need an expensive cable?
You need one built to the right specification, which is not the same as an expensive one. Cables certified to the relevant standard are widely available at ordinary prices, and beyond meeting the specification, price buys build quality and length rather than picture quality. Ignore anything sold on the claim of better colours.
Does a higher refresh rate use more power?
Yes, both at the screen and at the graphics hardware driving it, which is exactly why laptops reduce it on battery. On a mains-powered desktop monitor the difference is small enough not to be a deciding factor.
