Cause: Wi-fi calling is covering at home and masking the absence of VoLTE — Fix: This is a useful clue rather than a fix. It confirms the phone can carry voice over a packet network and that the problem is on the mobile side. Enable VoLTE.
Phone shows 4G but drops to no service during calls
Cause: The phone is trying to fall back to a 3G network that no longer exists — Fix: Enable VoLTE, then restart. If the setting is absent, the handset cannot do it.
A personal alarm has stopped calling through
Cause: The device used 2G or 3G — Fix: Contact the supplier immediately and ask for a compatible replacement. Many providers have replacement programmes. Do not wait to see whether it recovers — the person relying on it may not know it has failed.

Will my phone stop working entirely?
Not if it supports VoLTE, which every phone of the last several years does. The phones affected are older handsets, very cheap unbranded ones, and imports without UK network configuration.
Does this affect 5G in any way?
No, except that retiring 3G frees spectrum that networks are reusing for 4G and 5G. That is the reason it is happening.
